Initial commit: Caddy SIP Guardian module

Layer 4 SIP protection with:
- SIP traffic matching (REGISTER, INVITE, etc.)
- Rate limiting and automatic IP banning
- Attack pattern detection (sipvicious, friendly-scanner)
- CIDR whitelisting
- Admin API for ban management
